Breast Augmentation

Breast augmentation is a simple surgical procedure in which either implant/autologous fat is used to increase the size of breast to make it more attractive and aesthetically appealing. Apart from small breast if someone has minor sagging, which could have been caused by weight loss or post pregnancy, breast implants will not only enhance the volume but also lift up the breast to give younger appearance. However, in patients who have moderate to severe sagging only implants will not help and they need some other breast lift surgery/mastopexy. At Rejuvenaids, we routinely practice short scar breast augmentation, a technique made possible by the use of the Keller Funnel. In this technique scar is completely hidden in breast crease. As the incision is usually as short as only 3 cm which is almost invisible once stitch line is completely healed.

Size of implant to be used is an important decision influenced by:

• The desired end result

Some patients who already has undergone implant surgery and want to further augment their breast size will already have an idea of the size and type of implant they want, while others prefer to be guided by their plastic surgeon knowledge and experience. Sizers may be used to help in deciding implant size.

The choice of implant shape depends on many factors including:

• Patient choice
• Amount of natural breast tissue
• Desired final appearance/cleavage show

Breast implants available in market can be divided into two types:

• Anatomical (teardrop) – designed to be breast shaped, these implants are more suitable for women with smaller breasts or those who desires a more natural look.

• Round – these implants are often selected by women who already have a reasonable amount of breast tissue.

Having chosen the size and shape of the implant, the next decision is whether to go above the pectoralis muscle (sub glandular), Subfascial (above Pectoralis muscle but below pectoralis fascia) below the muscle (sub pectoral) or a combination of the two (dual plane).

Each approach to breast augmentation has its own pros and cons. Dr Neeraj will guide and discuss the implant placement with you to help you decide the optimal position for you, depending on your individual anatomy and preferences.

The most common widely used skin incision used is the inframammary incision, which is placed in the breast crease. This incision allows optimal positioning of the implant, and control of the ‘pocket’ where the implant is placed for best possible natural look.

Second most commonly used approach is incision around the nipple (peri-areolar) and is often used when breast enlargement is combined with a breast lift as it allows the nipple to be positioned higher on the breast.

A third option for skin incision is in the armpit (trans axillary). Using this approach means there are no scars on the breast – however, the final breast shape is more difficult to control as the surgeon has limited access when placing the implant.

Procedures we do

In this procedure unwanted fat is removed from other part of the body using liposuction (commonly from the abdomen, inner thighs). Harvested fat is re-injected into the breast to increase overall breast volume. This technique is suitable for women who desire a small increase in volume without the use of implants.

At Rejuvenaids, we also perform combined procedure in which we inject fat along with implant-based breast augmentation to provide more volume, natural look and better cleavage creation.

This technique is highly recommended in women who have had previous breast augmentation perhaps with too large implants where the skin has stretched and the edge of the implant is now visible/felt in the cleavage area.

This condition affects around 5% of women worldwide. It occurs due to defect during breast development and has several components of which some common findings are mentioned below:

The truly tuberous breast has a constricted base, deficient breast tissue usually in the lower pole, a raised or absent inframammary fold, and a widened areola with herniation of breast tissue in areolar region. It is one of the most common causes of breast asymmetry. Correction of tuberous breasts is not easy and needs a lot of expertise. It cannot be achieved by augmentation alone, as the constricted breast tissue must be released and redistributed to give more aesthetic and natural look. This is most commonly achieved through a peri-areolar approach which also allows control of the size of the areola and reduction of the herniated breast tissue to improve the final breast shape.

A common reason for seeking revision surgery is desire for bigger implant or capsular contracture which occurs to some degree in few implant-based breast augmentation surgeries.
Treatment for capsular contracture can involve removal of the capsule, exchange of breast implant and possibly a change of implant pocket in different plane. This may be combined with fat transfer for more appealing outcomes.

One of the most common demands from women requesting breast surgery is to lift the nipples to a more natural and aesthetically appealing position, majority of request for breast lift comes from mothers for saggy breast following breastfeeding. Breast ptosis post pregnancy is common even without breast feeding due to loosening of breast ligaments under hormonal influence.

Breast uplift surgery can be done through multiple techniques, depending on detailed assessment of patient and her desire. What are the different Breast uplift surgery – options are offered at rejuvenaids?

Peri-areolar mastopexy

Also known as ‘doughnut lift’, this technique is suitable for women who already have a good amount of breast tissue, good skin quality and only require the nipple to be lifted a small distance to achieve a pleasing breast look. The incision is made at junction of normal skin and areola so that its almost invisible.

Cresenteric mastopexy is a variant of this technique, where a ‘crescent’ of skin is excised above areola to lift it, leaving a scar around the top half of the nipple only.

Vertical scar mastopexy

If the nipple needs to be lifted more (usually >3 cm), Dr Neeraj may offer vertical scar mastopexy to you. Choosing a vertical scar (or ‘lollipop’ scar) technique allows the entire breast to be shaped and lifted apart from nipple, producing an aesthetically pleasing breast. The scar will be around the areola and vertically downwards towards the breast crease.

Inverted T mastopexy

Lifting and reshaping the breast using an inverted T or Wise pattern technique is most common type of breast uplift procedure which can correct almost all parameters required to create a more youthful and visually attractive appearance including:

  • Excess skin
  • Ptosis with significantly lower nipple areolar complex
  • Where the breast has lost a lot of volume

In this procedure final scar is placed around the nipple, vertically downwards and horizontally in the crease under the breast.

Augmentation mastopexy

This is procedure of choice for women who’ve lost a significant volume of their breast with ageing or for those who want an increase in the size of their breast as well as an uplift for ptosis. Augmentation mastopexy procedures requires meticulous planning which include detailed discussion with patient for choosing correct implant and procedure for best possible result.

‘Mummy makeover’ surgery is becoming more popular over recent years following increasing awareness of self- care post pregnancy as well women are more concerned about their appearance. This surgery package generally includes breast and abdominal surgeries undertaken on women following childbirth and lactation.

The increase in breast size during pregnancy and breast feeding can lead to a deflated and saggy appearance. Typically, this leaves the nipple and majority of breast tissue in a lower position which creates an empty look in upper part of the breast as well as poor cleavage show.

Abdomen is severely affected in women post pregnancy. It expands during pregnancy and retracts after birth, but unfortunately the abdomen doesn’t always return to its pre-pregnancy shape and size in most of the cases leaving an excess of hanging skin with lot of stretch marks.

Apart from external appearance, Internally the rectus abdominal muscles can become widened during pregnancy leaving a weakness in the abdominal wall and a bulging of the lower abdomen.
Tightening of the abdominal muscles along with removal of the excess lower abdominal tissue is carried out with abdominoplasty procedure. It is often combined with liposuction to sculpt the flanks and overall better contouring. This constitutes the second part of a ‘mummy makeover’.

In some women good result can be obtained even with a mini-abdominoplasty or liposuction alone if there is an only small amount of tissue need to be removed from lower abdomen.
